Forest End Medical Centre is located in the grounds of Sainsbury's supermarket, Bagshot Road with ample free parking opportunities. A large variety of services including Minor surgical services, Contraceptive services and NHS physiotherapy.
Balfron Practice is located in Bracknell town centre with limited free parking and direct access to a variety of services within the same building or adjacent buildings like podiatry, sexual health, a variety of clinics, xray and ultrasound facilities and more.

CHANGE TO MIDWIFERY SERVICES
With effect from 1st April 2012 midwifery services for
All existing services and clinics will be run at the surgery as normal and patients should not notice any difference to the service being provided.
There will be a designated midwife visiting the surgery.
If patients need to contact their midwife at any time other than in normal clinic, they may do so by telephoning the Community Midwives Office at Frimley Park on 01276 604241.

TRAINING PRACTICE
Patient Information
Balfron Practice & Forest End Medical Centre is currently applying to become a practice accredited to train doctors intending to become general practitioners.
To do this, the practice needs to be regularly inspected and this may include a review of patients clinical records. Doctors specifically approved for this purpose will carry out these assessments.
As part of this process, our GP that will be responsible for the training – (Dr. Sarah Lewandowski) - may sometimes wish to video record her consultation with patients that, in due course, will be used as part of the training for the trainee doctors.
You may therefore be requested to agree to such a video recording of your consultation.
Your permission will always be requested beforehand and you will be asked to sign a letter of consent. If after the consultation, you wish to withdraw such consent, this will be possible.
There is no obligation for you to agree to such recording.
